HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Who This Notice Applies To

This Notice of Privacy Practices applies to TrimDoc Health, Inc. and the licensed healthcare providers who treat you through the TrimDoc platform (“we,” “us,” or “our”). We are required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this notice.

How We May Use and Disclose Your PHI

Treatment

We use and disclose your PHI to provide, coordinate, and manage your healthcare. For example, your treating physician may share your PHI with a specialist, a laboratory, or a pharmacy when providing care.

Payment

We may use and disclose your PHI to bill for the services we provide. For example, your name and health information may be included in communications with payment processors as necessary to collect payment for your care.

Healthcare Operations

We may use and disclose your PHI for operational purposes such as quality improvement, staff training, legal compliance, and business management.

Other Permitted Disclosures

  • As required by law: We will disclose your PHI when required by federal, state, or local law.
  • Public health activities: We may disclose PHI to public health authorities for required reporting (e.g., communicable disease reporting).
  • Health oversight: We may disclose PHI to government agencies for audits or investigations.
  • Serious threats: We may disclose PHI to prevent a serious threat to the health or safety of a person or the public.
  • With your authorization: Other uses require your written authorization, which you may revoke at any time.

Your Rights Regarding Your PHI

Right to Access

You have the right to inspect and obtain a copy of your PHI. Requests must be made in writing to privacy@trimdoc.io. We will respond within 30 days.

Right to Amend

If you believe your PHI is incorrect or incomplete, you may request an amendment. We may deny the request if the information was not created by us, or if we determine the record is accurate and complete.

Right to an Accounting of Disclosures

You may request a list of disclosures of your PHI made by us in the past six (6) years, other than disclosures for treatment, payment, or operations.

Right to Request Restrictions

You may request that we restrict how we use or disclose your PHI. We are not required to agree to your request, except for disclosures to health plans when you pay out-of-pocket in full for a service.

Right to Confidential Communications

You may request that we communicate with you in a specific way (e.g., only by email, not by phone).

Right to a Paper Copy

You may request a paper copy of this Notice at any time by contacting us.

Our Duties

We are required to:

  • Maintain the privacy of your PHI
  • Provide you with this notice of our legal duties and privacy practices
  • Notify you following a breach of unsecured PHI
  • Follow the terms of the notice currently in effect

Changes to This Notice

We reserve the right to change this Notice and make the new provisions effective for all PHI we maintain. Revised notices will be posted on our website and available upon request.
